The purpose of this questionnaire is to better understand the strategies your organization uses to outreach and enroll local residents from communities of color. In particular, the Health Connector wishes to gain clearer insight on how Navigators are making inroads with specific ethnic groups and the additional resources and support that may be needed to take that work further so that health coverage inequities by race and ethnicity can be addressed and closed.
